PRE-CONSTRUCTION MEETING ACCKB PACKAGE 1

MAJOR ISSUE :
1. Requirement to obtained Letter of clearance/permission/no objection from Local Authorities( MPKKB, TNB, Jabatan Air Kelantan)
2. To submit Project Quality Plan - PQP, Safety Manual, Critical Path Method, CPM, financial projection, s-curve ( financial and physical progress )
3. To coordinate the works should be done by Main contractor-MISB, Redicare, HRPZ's sub contractor.

Package 2 : Presentation of Architecture and Medical proposal

The process of design a few discussions and presentations between the Main contractor and JKR's representative and KKM. On the date of 7th July 2008 :

The major changes :
1. To review of no of carparks - the total of carparks should be the no of new building requirement plus the no of carpark replacement on the existing area.
2. To make it same level between exixting OT room to the new OT room and should be link with corridors.
3. To provide bigger medical storage area for the whole Hospital operation. (Main medical record storage area ).
4. Reshuffle the clinical and surgical area.
5. Mechanical and Electrical to review and to reconsider the existing services.

PACKAGE 2 : NEW BUILDING PRE-BID DOCUMNETS

The Tender document collected at JKR office on 23 July 2008. Consist 6 Volumes:
Volume 1 : Contract and genereal requirement
Volume 2 : Architecture and Medical brief
Volume 3 : Civil works
Volume 4 : Structural Works
Volume 5 : Mechanical Works
Volume 6 : Electrical Works

The proposal should be submitted on 23 Sept 2008.

THE PROCESS OF LA




Letter of Award (LA) shall be prepare by JKR.
To able the LA will issued to Contractor, a few document should be prepared by relevant parties to able a LA issue to contractor.
1. A Letter of Price Acceptance By Contractor ( base on the final negotiation meeting )
2. A letter from Treasury
3. A letter of Turun Kuasa from Ministry of Works to Ketua Pengarah of JKR.

FINALIZATION OF BQ & CSA AND FINAL NEGOTIATION

After the process of final finalization of contractor proposal, Contractor reproduce the BQ and Contract Sum Analisys (CSA) and submit to JKR CKUB for the price realization ( we call it PRE-NEGO) then process of negotiation process. Several BQs and CSAs have been produce to match the JKR requirement and pricing.

At the date of 28 August 2008, Meeting of the final negotiation held at JKR HQ.
The meeting took only 30 minutes and We agreed that
1. The contract for Package 1 is 8.975M.
2. The Contractor will write officially that agreed the contract amount is RM 8.975M
3. JKR will prepare the LA to contractor.

2. Scope of work package 1 is stand alone contract.
3. Earlier contractor proposal for New Pejabat Pengarah accepted with minimize renovation works.
4. Structure and Civil work required to conduct Dilapidation report before and after works.

NCR ISSUE

NCR ISSUE
Date of Issued : 20 May 2008
Issued by : JKR CUB

NCR is Non Compliance Report. Means the proposal is not compliance in:

1. No proposal or doesn't say - not mentioned in early proposal
2. No comply to specification and requirement.
3. Proposed material or specification which are not in approved list by JKR/KKM.
4. Not comply to EPU requirements.

The first contractor proposal, JKR and KKM list a few significant NCR issue.

1. The proposal of Decanting works for Pejabat Pengarah and relocation of New Pejabat Pengarah. (Quarters Block A)with 2 option.
a. to relocate at the ground floor of the quarters blocks instead using the upper floor units. or
b. to maintain existing Pejabat Pengarah.

2. To proposed fibre optic as the option. - confirmation will be decide when the stage of Final Negotiation with KKM, MOF and JKR.

3. Internal building layout for Respiratory Unit and Ward 6.

4. Others technical issue. ( refer Minutes of meeting no 2 )

The contractor required to comply in the second proposal with 2 weeks from the date of issuance of NCR.

Attended : Represent of Contractor, Consultants, HOPT, all of HODTs, HRPZ II, Redicare, JKN, KKM, CUB.

ISSUE :-
1. The issuance of NCR to Contractor a week after presentation.
2. Changes of the Scope of Works : Section 5 Temporary of Carpark - Omitted
3. The "layout and flow" of Respiratory Unit. - required to have Room Data Interaction.
4. The Convertion of Accomodation to be Pengarah Office : Contractor to reviews
5. The usage of HEPA Unit for specified room.
6. The usage of AC Unit for specific area: refer guideline and the entitlement of room.

All the issues from HODTs and KKM will mention further in the NCR List. (Tentative date by 20th April 2008)

This is discussion request by Contractor MISB to present thier technical proposal.
The issue :
1. The scope of Decanting work :- should be 1 to 1 basis. Means all the space, services, medical equipments and furniture shall be relocated. No new items, no new space or no new equipment except it allowed and approved by KKM.

2. Fibre-optic are required, contractor shall be priced it in the special/option item.

3. All Consultant shall discuss with HODTs to minimize Non compliance Report (NCR)

DECANTING WORKS ( PENGOSONGAN BANGUNAN)

PENGOSONGAN BANGUNAN (DECANTING WORKS)

JKR menitikberatkan perkataaan "DECANTING WORKS" lebih kepada mengosongkan bangunan dan pemindahan ruang, fasiliti dan juga peralatan pada bangunan yang akan dirobohkan kepada ruang yang dicadangkan oleh pihak HRPZ II.

Kontraktor berpendapat perkataan DECANTING WORKS itu adalah skop kerja untuk kerja -kerja menaiktaraf fungsi bangunan terutama dari pada kuarters/apartment kepada ruang pejabat. dan dari Unit Sajian ( dapur ) kepada klinik dada yang mempunyai keperluan rekabentuk yang lebih besar ataupun atau pun keseluruhannya bertukar.
Terbahagi kepada 3 bahagian :
1. Ujian Penilaian Keadaan (Condition appraisal test)
2. Pengubahsuaian (Renovation)
3. Pengosongan Bangunan
